Job Description:

Warrington NHS Foundation Trust is seeking compassionate and dedicated Health Care Assistants to join our community health teams. This role involves providing essential care and support to patients within their homes, supporting the delivery of clinical and personal care, and promoting health and independence among community members. The successful candidate will work closely with registered nurses and other healthcare professionals to deliver high-quality care tailored to the needs of each patient.

Responsibilities:

• Provide personal care to patients in their homes, including assistance with daily activities, personal hygiene, and mobility.
• Monitor and record patients’ health conditions as directed by healthcare professionals, such as measuring vital signs and noting any changes in behavior or condition.
• Support patients with medication management under the guidance of healthcare professionals.
• Assist with the planning and implementation of care plans.
• Offer emotional and practical support to patients and their families.
• Ensure that all care is provided with respect for patient’s privacy and dignity.
• Work collaboratively with other members of the healthcare team to ensure a coordinated approach to patient care.

Requirements:

• NVQ Level 2 or 3 in Health and Social Care or equivalent is highly desirable.
• Previous experience in a caring role, preferably within a community setting.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a diverse team.
• A compassionate and professional approach to patient care.
• A valid driver’s license and access to a vehicle may be required due to the nature of the community-based role.
