Job Description:

University Hospitals Plymouth NHS Trust is seeking an innovative and efficient Virtual Ward Coordinator to oversee the operations of our virtual wards. This remote position plays a crucial role in managing the day-to-day administrative tasks, coordinating patient care, and ensuring seamless communication between healthcare providers and patients receiving at-home care.

Responsibilities:

• Coordinate the admission, discharge, and ongoing management of patients on the virtual ward.
• Serve as the primary point of contact for patients, healthcare professionals, and external agencies involved in patient care.
• Schedule and facilitate virtual consultations and follow-ups between patients and healthcare providers.
• Maintain accurate and comprehensive patient records within the electronic healthcare system.
• Monitor patient progress and coordinate care plans with multidisciplinary teams.
• Ensure compliance with all clinical governance and patient safety standards.
• Provide administrative support for the virtual ward team, including managing schedules, organizing meetings, and handling correspondence.

Requirements:

• Proven experience in healthcare coordination, nursing, or a related administrative role within the NHS or a healthcare setting.
• Strong organizational and multi-tasking skills, with the ability to manage complex schedules and workflows.
• Excellent communication skills and proficiency in virtual communication platforms.
• Familiarity with electronic health records and data management systems.
